Supervision: Case Manager Supervisor General Summary: The Housing Case Manager is a full-time position that provides comprehensive case management services to survivors of human trafficking. This position will work in collaboration with the Sexual Assault Center and Eye Heart World to ensure that the survivors needs are met. ESSENTIAL DUTIES AND TASKS: • Make certain that all Newcap Housing Programs are Housing First so that people are housed quickly without preconditions or service participation requirements. • Confirm that all Newcap Housing programs are person-centered and safeguards client choice. Choice can include location and type of housing, level of services, and decisions related to the type of other supportive services. • Work with the Sexual Assault Center and Eye Heart World to broaden advocacy response to human trafficking • Serve as the agency representative to local, regional, and state level human trafficking coordinating bodies • Attend program-related community, coalition and committee meetings, as assigned. • Maintain and update resource and referral lists specific to the topic of human trafficking • Participate in regular meetings with the Case Manager Supervisor • Provide crisis intervention services during business hours to clients who have been trafficked, as needed (some afterhours interventions may be necessary) • Provide case management and referrals to clients who have been trafficked, as needed • Advocate with other agencies on behalf of clients who have been trafficked, as needed • Prepare case management related reports including but not limited to outcomes, successes, etc. • Actively participate in client- and program-related meetings and training, including staff meetings. • Maintain relationships with services providers, volunteers, staff and community members. • Respond appropriately to a client a crisis • Use a variety of methods to offer short term (up to 24 months) housing assistance to individuals or families who have experienced human trafficking • Implement a thorough assessment with client, including a goal-oriented care plan. • Develop realistic action plan with the client using autonomy • Provide case management duties for at least 13 clients at any given time • Update case management and client files weekly • Maintain clear, sufficient case notes on all clients. • Collect and organize data into appropriate format for the Case Manager Supervisor and funders' database. • Enter all clients into the CAPTAIN database in a timely manner. • Make sound and timely decisions under pressure. • Recognize what is the most suitable in particular situations. • Effectively make decisions in non-standard situations. • Exhibit independence, decisiveness, and responsibility. • Clearly and persuasively communicate decisions and order to avert crisis. • Prioritize and plan work activities • Use time efficiently and effectively • Think carefully about the likely effects on others of one's words, actions, appearance, and mode of behavior. Select the words or actions most likely to have the desired effect on the individual or group in question • Observe safety and security procedures, and use equipment and materials properly • Support and promote an environment that appreciates and holds opportunities for all • Participate in the bi-annual Point-In-Time count in January and July each year. • Conduct periodic follow-up assessments on schedules prescribed by prevailing state, federal and agency rules, regulations and policies. • Provide clients with information about community resources, homeless programs and housing resources and assist them in identifying, evaluating, and connecting to those that best match their preferences. • Work collaboratively with Case Manager Supervisor, Housing Director, with other agency's staff, to meet program goals and performance targets. • Review/Audit of client files to ensure case management plans address identified needs, are up-to-date and that data quality is maintained. • Maintain related administrative record keeping and reporting systems as required by contracts. • Adhere to all grant requirements. • Participate in training opportunities • Adhere to boundaries and professional ethics. • Conduct crisis intervention as needed. • Other duties as assigned. WORKING CONDITIONS: This work is generally performed in an office environment. Much of the work will take place in the field across the county. Some overnight is required. EFFORT: Long periods of time working at a computer terminal. Use of keyboard may be stressful to hand or wrists. Must be able to see, hear, and speak. Frequent handling and fingering, reading, working with information, standing, walking. Occasional lifting up to 25 pounds, reaching, climbing, stooping. Minimal evening and weekend are required. MACHINES, TOOLS, EQUIPMENT: Computer, cell phone, printer, photocopy machine, fax machine, calculator, shredder, telephone, van or car. MINIMUM QUALIFICATIONS: • Bachelor's Degree in Social Work or human-service related field. Work Experience may substitute required education on a year-by year basis. • Knowledge and experience in the field of human trafficking, domestic violence, dating violence, sexual assault, and stalking preferred. • Experience working with homeless individuals and families is desired. • A basic understanding of poverty, its causes and conditions and its impact on individuals and communities is essential. • Willing to work non-traditional hours and days to meet the needs of the position. ABIILITIES REQUIRED: • Ability to read, write using proper grammar/punctuation, and communicate effectively • Possess organizational, planning and analytical skills • Proficient in Microsoft Office Suite • Experience with word processing, spreadsheets, and public speaking. • Ability to perform tasks with minimal supervision or over-site. • Effective verbal and written communication skills. • Ability to inspire and motivate clients and help them overcome barriers SPECIAL REQUIREMENTS: • Must possess a valid driver's license. • Must pass a criminal background check. • Must pass physical exam as it pertains to the job description. • Must be able to maintain the confidentiality of any information encountered.
