Willow Creek Behavioral Health is a new 72 bed psychiatric hospital. This state-of-the-art facility underscores our commitment to individuals and families seeking support and treatment with debilitating psychiatric concerns. Upon completion of orientation and under general supervision, Assessment & Referral Specialist performs assessments at Willow Creek Behavioral Health, as well as outside the facilities, depending on the needs of the patient or hospital, screens admissions for clinical and medical appropriateness and communicates this information to a physician for determination of admissions and level of care direction. Works closely with business office, nursing and clinical staff as well as external parties to ensure the timely admission of patients. ESSENTIAL JOB FUNCTION AND RESPONSIBILITIES Customer Service In-person and telephone communication is consistently attentive, responsive and reflects "helpfulness" -. Professional awareness and boundaries are observed. Unnecessary self-disclosure is minimized. Telephones are answered by the third ring whenever possible and a professional greeting. Dedicated extensions/mobile phones with voice mail have a personalized greeting, and messages retrieved/responded to in a timely manner. Critical customer encounters are met with advanced customer service skills and resolved. Assessment Services Coordinates activities related to admission of patients and appropriate follow-up activity. Conduct initial evaluation to include interviewing patient and/or patient's representative to obtain necessary personal, medical and psychiatric history and financial data to determine eligibility for admission, appropriate level of care and document recommendations and Axis I-IV diagnoses, as appropriate. Confers with physicians, nursing, housekeeping, transport and other staff members to coordinate and schedule admission of patient. Assigns accommodations based on physician's admittance orders, nature of illness, availability of accommodations and other information. Prepares record of admission, transfer and other required data. Notifies departments of patient's admission. Maintain record of admissions and discharges and complies occupancy-census data. Leadership Continuously self regulates professional presentation especially during emotional charged situations. Presents informed and available to medical staff during patient rounds or telephonic consultations. Partners with others to ensure cohesive teamwork and healthy communication. Gives consideration to situations that may produce divisiveness or be received as diminishing others role or authority. Medical Records and Confidential Communication Appropriately considers the sensitivity and confidentiality of medical/psychiatric information along with the need-to-know before sharing information with others. Medical records of deficiencies are minimized, and when identified, corrected as soon as practical. Maintain files in a neat, orderly, and confidential manner with special attention to security. Child (age 1 - 12 years) Adolescent (age 13 - 17 years) Adult (age 18 - 64 years) Geriatric (age 65 years +)Qualifications Education / Licensure: Candidate will possess an accredited master's degree in behavioral sciences or psychiatric nursing, and an unencumbered license issued by an appropriate state board. LPC, LMFT, or LCSW highly preferred. A minimum of two years in a psychiatric or mental health setting desirable. Must have a valid driver's license. For candidates who are RN, candidate will possess a valid current registered nursing license from the Board of Nursing or from a nurse compact state, where applicable. Skills: 1) Comprehensive understanding of the behavioral healthcare industry and experience in clinical interviewing, patient assessment, family dynamics, treatment modalities and planning and admission requirements using the established hospital criteria. 2) Thorough knowledge of JCAHO, CMS, third party payors, managed care organizations and state and federal regulatory standards/requirements. 3) Ability to communicate professionally and effectively, orally, in person, and in writing, with multidisciplinary team members, managed care organizations and business office, providing needed information in a logical, concise manner using technical language that accurately describes patient's condition and need for hospitalization. 4) Ability to organize and prioritize workload to meet deadlines, orchestrate a myriad of activities and to manage multiple priorities within complex and changing systems; 5) Able to self-regulate and facilitate helpful management dynamics during stressful situations. Additional Requirements: Timely renew and maintain cardiopulmonary resuscitation for healthcare professional's certification, professional skills verification and "Handle with Care" - (psychiatric crisis management) certification. If candidate is a registered nurse, timely renew and maintain an active registered nursing license in the state of WI. Must have reliable transportation and be able to retain automobile liability insurance at the minimum level required by state law. Must have thorough knowledge of how to access local resources, WI commitment laws and the WI Mental Health Code.
