Evergreen is seeking compassionate and talented person to join the Nursing team!

Evergreen's 80-bed Medicare-certified licensed skilled nursing facility offers short-term rehabilitation care, long-term care and palliative care. Creekview long term skilled nursing provides care for 58 residents in six households with a nationally-recognized innovative approach. Creekview Rehabilitation Center at Evergreen provides care for 22 residents who are focused primarily on short-term rehabilitative therapies and who plan to return home after recovery.

This position is 7 shifts per week on the NOC shift( 10:30am-6:45am). Every other weekend and every other holiday are required.

This position is eligible for Evergreen's vision and dental insurance, and 403(B) retirement program with company match. This position is also eligible for paid vacation and sick time.

  • Perform skilled nursing tasks
  • Assure proper techniques and/or procedures are followed
  • Supervise unit staff during assigned shift
  • Use appropriate safety procedures and standard precautions Minimum Qualifications:
    
    • Valid Registered Nurse or Licensed Practical Nurse license in the State of Wisconsin
    • Current CPR certification
    • A desire to work with older adults
