Description:Registered Nurse (RN)The Float Registered Nurse (RN)
is responsible for providing quality professional nursing care to
patients and their families out in the community. The
responsibility includes providing a safe environment for patients,
families, staff, and visitors and working collaboratively with the
multi-disciplinary care team to provide the best outcomes for the
patient and family.Schedule: 4x a week 7:00 am - 4:00 pm, minimum
every 4th weekend, on-call 1 shift every 5-6 weeks, 2 holidays per
yearKey Job Duties of the Registered Nurse (RN):1. Provide clinical
and supporting case management based on patients diagnosis and
end-of-life symptoms.2. Complete initial, comprehensive, and
updated assessments of patient and family needs.3. Develop,
implement and review care plans for patient and family. Update care
plan as the needs of the patient and family change4. Prepare for
and participate in interdisciplinary team meetings as scheduled5.
Communicate with all interdisciplinary team members6. Report
appropriate findings and communicate significant changes to
attending physician and processes physician/NP orders7. Teach
family members patient care skills and encourage their
participation including, but not limited to, pain control,
medication management, dressings, hygiene and nutrition8.
Administer and titrate medication according to physician/nurse
practitioner orders10. Prioritize daily work and make adjustments
taking into account the needs of patients, Unity and other staff11.
Complete new patient admissions as neededKnowledge, Skills, &
Abilities of the Registered Nurse (RN):Build relationships with
referral sources and organize referralsCommunicate with all
interdisciplinary team membersPromote and maintain
confidentialityMaintain personal and professional
boundariesMaintain regular and reliable attendanceShares in
providing 24-hour, seven-day-a-week coveragePrioritize daily work
and make adjustments taking into account the needs of patients,
Unity, and other staff.Comply with all applicable local, state, and
federal laws and regulations as well as policies and procedures of
Unity.Satisfy mandatory training requirements regarding local,
state, and federal regulations and/or Unitys policies and

nursing required, Bachelors degree nursing preferred.RN license in
