Job Description:

Provides case management for specific patient populations, utilizing clinical expertise, communication, and problem-solving skills to optimize outcomes.

Performs patient needs assessments, facilitates referrals, and links patients to health and post-acute resources across the care continuum.

Promotes interdisciplinary collaboration, care coordination, and appropriate length of stay, ensuring compliance with discharge regulations.

Collaborates with Utilization Review Nurses, reviews patient statuses daily with social workers, and documents care plans accurately.

Facilitates referrals for high-cost medications, coordinates care transfers, and advocates for patients and families.

Uses predictive analytics, manages medication needs, and communicates post-acute care requirements during transitions.

Reqs: RN license in Virginia, Bachelor's degree or equivalent experience, 3+ years clinical case management, strong communication, and teamwork skills.

Full-time, inpatient role with day/evening shifts, promoting innovative, patient-focused care.
