their families.The Primary Registered Nurse is responsible for
being the primary case manager for patients who live in the
community and within facilities. The primary nurse collaborates
with float nurses to provide safe, consistent, and compassionate
care to both the patient and their family. Flexibility, teamwork,
and exceptional communication are a must for the primary nurse. The
primary nurse collaborates with the interdisciplinary team to
provide updates and to achieve high-quality patient standards.
During periods when your caseload is lighter, you will be expected
to support your team by assisting with patient visits and
transitions, serving as a Float Registered Nurse as needed. This
collaborative approach ensures that all patients receive the
highest level of care, regardless of fluctuating caseloads. This
role is in the Sturgeon Bay territory and may travel outside of
their territory at times.Schedule: 36 hours per week - 9-hour
shifts, 7am-4pm or 8am-5pm , back up rotation - one shift every
10-15 weeks, minimum every 3rd weekend but currently working every
8th (12 hour shifts), roughly 2 holidays per year.Key Job Duties:1.
Collaborate with primary nurse to help provide clinical and
supporting case management based on patients diagnosis and
end-of-life symptoms.2. Complete initial, comprehensive, and
routine assessments of patient and family needs.3. Respond to
urgent patient/family needs in collaboration with triage nursing
staff4. Initiate crisis care as needed5. Develop, implement and
review care plans for patient and family. Update care plan as the
needs of the patient and family change6. Report appropriate
findings and significant changes to the primary team, attending
physician and primary caregiver7. Communicate with all
interdisciplinary team members8. Teach family members patient care
skills and encourage their participation including, but not limited
to, pain control, medication management, dressings, hygiene and
nutrition9. Provide education to patient/family members on disease
progression, symptom management, medication and other associated
patient care needs10. Administer and titrate medication according
to physician/nurse practitioner orders11. Record all received
orders to the Electronic Medical Record and initiate follow up as
needed12. Delegate nursing duties to LPN and supervise care
provided by CNA13. Prioritize daily work and make adjustments
taking into account the needs of patients, Unity and other staff14.

degree in Nursing required, bachelors degree preferred.2. RN
license in the state of practice required (RN must practice under
the law and regulations governing nursing in the state of
