Job Description:

seeking a dedicated and compassionate Patient Navigator to join our
team. This pivotal role supports patients through their healthcare
journey, ensuring seamless coordination of care and optimal health
outcomes. By applying innovative solutions in the delivery of care,
the Patient Navigator plays an essential role in our mission to
empower communities and transform healthcare provision. You will
serve as a critical link between patients and healthcare providers,
facilitating communication, education, and resource connection
based on individual patient needs. Serve as the primary point of
contact for patients, providing consistent support throughout the
treatment process Coordinate patient care by scheduling
appointments, managing referrals, and ensuring timely access to
healthcare services Educate patients about their health conditions,
treatment plans, and any necessary lifestyle changes Collaborate
with healthcare providers to tailor care plans to individual
patient needs and monitor outcomes Connect patients with community
resources and support systems to enhance their health and
well-being Maintain comprehensive and confidential patient records
and provide regular updates to healthcare teams Implement patient
satisfaction measures and participate in continuous improvement
initiatives to enhance service delivery and patient experience
Strong interpersonal and communication skills Ability to manage
multiple tasks and prioritize effectively Knowledge of healthcare
systems, medical terminology, and care coordination Competency in
electronic health record (EHR) systems and data management Empathy
and cultural sensitivity to work with a diverse patient population
