Willow Creek Behavioral Health is a new 72 bed psychiatric hospital. This state-of-the-art facility underscores our commitment to individuals and families seeking support and treatment with debilitating psychiatric concerns.Performs mobile assessments for Willow Creek Behavioral Health, depending on the needs of the patient or hospital for ages 5 years and older. Screens assessment admissions for clinical and medical appropriateness and communicates this information to a physician who makes the admissions and level of care decision.ESSENTIAL JOB FUNCTION AND RESPONSIBILITIESTHE ESSENTIAL JOB FUNCTIONS ARE CATEGORIZED ACCORDING TO TJC RECOMMENDATIONS.Departmental ResponsibilitiesFacilitate the response to requests for services, including the initial response, the assessment and referral process, the designation of appropriate level of care, initiation/admission into services, and appropriate follow up activity.Provide initial evaluation and initiate the integrated assessment, documenting patient symptom logy/functioning/needs.Staff each assessment with appropriate physician, documenting recommendations and Axis I-V diagnoses, when appropriate.Advocate for the lowest level of care consistent with patient needs. Consult with supervisor in the planning, development, implementation, and evaluation of departmental activities.Demonstrate flexibility with work schedule to help meet the needs and demands of the patient population/census and subsequent adjustments.Provide all other duties as assigned.Dependability/InitiativeFunctions as a role model for other department and facility staff.Supports facility-wide quality/performance improvement goals and objectives.Participates in Performance Improvement activities, as assigned, and is responsible for the effectiveness of the Performance Improvement program as it pertains to Assessment & Referral.Demonstrate ability to operate in an unstructured environment, orchestrate a myriad of activities and to manage multiple priorities within complex and changing systems.Education DevelopmentAttend all mandatory facility in services and staff development activities as scheduled.Completes annual updating of competency file (attends Competency Day as scheduled).Identify personal learning needs to assist with staff development and assist with planning and teaching staff development programs, as assigned.Hospital Policies and ProceduresServe on Facility Committees as assigned.Assist with evaluation of department goals and objectives with the participation of staff on a regular basis.Ensure that personal performance meets TJC, Medicare and state regulatory requirements.Ensure that all deficiencies identified through the Performance Improvement plan are corrected with appropriate problem-solving actions. Adhere to facility standards concerning conduct, dress, attendance and punctuality.Maintain confidentiality of facility employees and patient information.Complies with established measures for infection control, OSHA regulations, and risk management.Adhere to all facility and personnel policies and procedures.Complies with all facility policies and procedures related to ethical practices and conduct.Safety ManagementPractice standard precautions and environmental safety guidelines.Follows/implements all safety policies and procedures during actual or staged safety emergencies (fire, tornado, disaster, etc.). Adhere to all facility safety management policies, including maintaining compliance with environment of care modules.Ensure that the physical environment of the area is monitored for safety, cleanliness, and attractive appearance and all deficiencies are corrected in a timely manner. Willow Creek Behavioral Health encourages all registered nurses to obtain a bachelor’s degree in nursing or in the behavioral health sciences OR meet requirements for national certification as a psychiatric nurse within three years of employment. Candidate will possess a valid current registered nursing license from the Wisconsin Board of Nursing or from a nurse compact state, where applicable.Experience:Minimum of two years in direct clinical experience in a behavioral health/chemical dependency setting. Minimum of two years of ER/ICU/Critical Care or acute psychiatric experience.Additional Requirements:Experience in patient assessment, referral, treatment planning and communication with external review organizations or comparable entities. Completion of CPR/AED course and completion of Crisis Prevention Intervention (CPI) training, including appropriate and safe use of seclusion and restraint, within 30 days of employment. Thorough knowledge of TJC, HCFA, OSHA, state and federal regulatory requirements. Must have reliable transportation and be able to retain automobile liability insurance at the minimum level required by state law. Must have thorough knowledge of how to access local resources, Wisconsin commitment laws and the Wisconsin Mental Health Code.
