COMPENSATION AND BENEFITS Dentist (DDS / DMD)

• Up to $195,267 compensation package
• 401K Match, 100% match up to 4% employee contribution, another 50% match toward 5% - 6%
• 25 Days PTO in year 1, jumps to 30 days PTO year 2 + Holidays
• FQHC - Approved site for Federal School Loan Assistance program

RESPONSIBILITIES AND FACILITY DETAILS Dentist (DDS / DMD)

• Federally Qualified Health Center setting (FQHC) - Qualified for Federal school loan assistance
• General Dentistry - Variety of case-mix including crowns, bridges, endo, and dentures
• Monday-Friday Schedule
• H1B Visa Sponsorship available for providers with DDS/DMD from a 4 year Dental Program in US.
